When we process
your order, we'll send you a welcome letter that includes the Online Desk Reference
web address, your access ID and password, and guidance on how to get the most out
of this important resource.

| IMPORTANT: Your subscription to the ONLINE DESK REFERENCE will
be renewed after each full year of service, unless you decide to cancel. If you provide
a credit card when making your original purchase, the renewal will be charged to
that card on or within two weeks of the expiration date of your subscription. If
you paid for your original order by check, an invoice will be sent for the renewal
amount. Please note that you can cancel your subscription at any time and for any
reason. If there is time remaining in your subscription term when you cancel, you
will receive reimbursement for each remaining full month. (For example, if you cancel
six months into your annual subscription, you will receive reimbursement for the
six remaining months.) Refunds can be in the form of a credit to the credit card
or a check from Tom Terez Workplace Solutions Inc. All transactions in U.S. currency. |

The four access packages differ in terms of the number of allowed users, the
annual fee, and payment method.
For payment purposes, the Online Desk Reference works like a subscription. You pay
for a full year of access. At the end of the year your subscription is renewed. Your
satisfaction is completely guaranteed, and you can cancel at any time, no questions
asked. If you cancel, you receive reimbursement for each full month remaining
in your access term.
As an authorized user, you have full access to the Online Desk Reference, and you
can download, print, photocopy, and use any of the download-ready materials (5-Minute
Assessments, 22 Keys Workplace Assessment, articles, etc.). If you purchase the Group
Access package or one of the other packages for multiple users, you are permitted
to circulate the access ID and Password to the maximum allowed number of people.
They too have full access and can use all of the downloadable materials. Others in
your workplace who do not have access are not permitted to make photocopies of the
materials, but they can complete the assessments and other tools, as long as an authorized
user has made and circulated the copies.
